The Proposed Logica Tier-2 Showcase at HIMSS 2015

Goals

  1. Identify and demonstrate compelling use cases arising from Logica and associated orgs that are driving need for enhanced interoperability services
    • These include:
      • Care coordination, workflow optimization, care plan-based CDS
      • Connected care, with patient-centered and mHealth/biosensor services, and integration of patient data with EHR data
      • Pop-health-based patient management, personal CDS based on patients-like-mine retrievals, dashboards and summaries of provider-specific and practice-specific statuses
    • Description of overall services needed and architecture for achieving
    • Description of sandbox and role in providing:
      • Development testbed for apps
      • Assembled resources needed (such as middleware services, test data and data sources, optional services for design, usability testing, technical support, compliance testing, deployment pathway support, etc.
  2. Showcase 3 specific projects, focusing mostly on care coordination category (see above) but possibly showing dashboards from pop-health category
    • Project details
      • Mayo Clinic post-op management against care plan
        • Assembly of needed info per patient for encounter (e.g., rounding on day 2)
        • Identification of appropriate care plan and position on it, as well as need to shift to new care plan
        • Generate potential orders/actions to be done, e.g., blood test or med order
        • (Possibly) provide dashboards of patients by status, by provider and by practice
      • LSU/VA ambulatory diabetes management - Functions to be defined by LSU/VA – possibly similar to Mayo project above
        • Assembly of needed info per patient for encounter (e.g., office follow up or specialty referral)
        • Identification of appropriate care plan and position on it, as well as need to shift to new care plan
        • Generate potential orders/actions to be done, e.g., insulin med dose change or lab test request, or referral for ped exam
        • (Possibly) provide dashboards of patients by status, by provider and by practice
      • Intermountain Healthcare pulmonary embolus management – need IHC info here, possible use of same functionality as above
        • Diagnosis/workup care plan application for patient with suspected PE
        • Recommendation for CT angio or other, heparin, etc.
        • Integration with care plan for ordering tests and following up care
    • Demo
      • Provide an overview of the project
      • Focus on a particular scenario through each project, starting at a point where patient data needs to be assembled, evaluated against a care plan/decision support model, and an action (or set of actions/order set) is proposed
      • Select the action and have it trigger appropriate HL7 message (e.g., order set request)
      • Highlight the common infrastructure services needed
      • Describe the approach to providing them
      • Emphasize:
        • (1) The commonality among the use cases for shared services
        • The ability (2) to develop the app interoperably, (3) to run on other systems, (4) to use data from different sources
